Re-Certification

RE-CERTIFICATION REQUIREMENTS:

CHSP diplomats must be re-certified every 5 years. CPSO diplomats re-certify every 3 years.  The re-certification requirements include documented hours of both professional practice in safety related activities and contact hours of continuing education in safety related subjects. Each CHSP must document 75 hours, if fully employed in healthcare. Each CPSO must document 45 hours if fully employed in healthcare. Download and fill out form below and send in with $35.00 re-certification fee.

** The Board will notify each diplomat by letter with re-certification requirements. Failure to re-certify could result in revocation of the certification.
